Change of Name - Adult
A person desiring to legally change their name must be a resident of Miami County for at least 60 days immediately prior to the filing of the application for the change of name.

In addition to the forms listed below, applicants must present the following documents at the time of filing, the Court must see the original documents:
- Valid photo I.D.
- Certified copy of the Birth Certificate
Once a name change has been finalized certified copies will be provided to the applicant. It is up to the applicant to send a copy to the Vital Statistics Office to update the Birth Certificate.
