2025 1095-C forms were sent upon request, in accordance with the Paperwork Burden Reduction Act (H.R. 3797) update which became law on December 23, 2024.

If you would like to request a copy of your 2025 1095-C form, you may do so using this link. You have the option to pick up the form from Human Resources or have it mailed to your home address.
